Specialist Management of Complex and Revision Eye Surgery

Revision eye surgery involves the management of complex conditions and complications arising from previous eye surgery, trauma, or underlying structural abnormalities of the eye.

These cases require advanced surgical planning, highly specialised techniques, and a tailored approach to each individual eye.

Many patients are referred by other ophthalmic surgeons to Mr Sabatino for expert management of challenging conditions, reflecting established trust and experience in complex and high-risk surgical cases, including complications of previous procedures and advanced reconstructive techniques.

Conditions Treated

Mr Sabatino provides specialist care for a wide range of complex anterior and posterior segment conditions, including:

• Dislocated natural lens (crystalline lens)
• Dislocated or unstable intraocular lens (IOL)
• Aphakia (absence of the natural lens)
• Complications following cataract surgery
• Complications following vitreoretinal surgery
• Complex lens instability with reduced capsular or zonular support
• Iris defects due to trauma or previous surgery

Each case is carefully assessed to determine the safest and most effective surgical strategy.

Secondary Lens Fixation Techniques

When standard intraocular lens support is not present, specialised techniques are required to restore stable vision.

Mr Sabatino offers a full range of modern secondary intraocular lens fixation techniques, selecting the most appropriate method based on the individual anatomy of the eye:

• Scleral-fixated intraocular lenses, including advanced techniques such as: Sutureless fixation (e.g. Yamane technique), Gore-Tex–sutured scleral fixation, Carlevale intraocular lens implantation.
• Iris-fixated intraocular lenses, where appropriate.

These techniques allow secure lens positioning while minimising surgical trauma and optimising visual outcomes.

Iris Reconstruction and Artificial Iris Implantation

For patients with iris defects, whether congenital, traumatic, or following previous surgery, reconstruction can significantly improve both visual function and comfort.

Mr Sabatino offers surgical iris repair and reconstruction, and implantation of custom artificial iris devices.

He is certified in artificial iris implantation and provides a tailored approach to improve light sensitivity (photophobia), glare, and cosmetic appearance.

A Personalised Surgical Approach

Revision eye surgery is highly individual.

Each patient undergoes a comprehensive assessment and detailed discussion of the available options, allowing a surgical plan tailored to the specific anatomy and visual requirements of the eye.

Where appropriate, different techniques may be combined to achieve the safest and most effective outcome.
